Long-Term Disability

General Information
Employees are automatically enrolled in a plan which provides monthly income of two-thirds (2/3) of the employee’s annual contract salary if you are disabled due to illness or injury for more than 60 days.  Your labor agreement or individual contract will list your required contribution towards long-term disability coverage, if any. National Insurance Services is the broker and administrator for Madison National Life, the current the long-term disability carrier.

If you are submitting a claim for long-term disability please contact Human Resources so that an employer claim form can be submitted by the District.
