• Submit the form at least two weeks before the requested usage date
• If you submit your form in person at an ISD 199 building, please include the $10 application fee with your application form (The fee increases to $20 if you are submitting your form less than eight days before the requested usage date).
• *ISD 199 employees should use the Facility Use Permit Application listed on the Employee Forms webpage.
